Thank you for participating in the Health Risk Appraisal (HRA) offered through your company.  The Health Risk Appraisal will help you better understand your overall health and guide you with ideas and suggestions for areas of improvement.

Please complete all questions to the best of your knowledge.  There is a time limit on each page of 5 minutes of the HRA to protect your information from being viewed by others if you are not at your computer.  If you are not able to complete the HRA, each page you complete will be saved for the next time you log-in.  

You will receive a Report Card and Healthy Employee Life Plan (H.E.L.P) in the mail with your results once you have attended the Health Risk Assessment Event.
