The mission of LivingWell Health Solutions is to help individuals achieve positive, lasting improvement in their health. In order to take steps to improve their health, individuals must be aware of their specific health risks. Through the LivingWell Health Solutions HealthSCREENING, personal health profiles are defined and emerging risk conditions are measured.
The LivingWell Health Solutions HealthSCREENING is an on-site health screening that provides each participant with highly individualized and pertinent information regarding their current health status. There are three parts that make up the Personal Health Evaluation.
1. Health Risk Assessment Questionnaire - Individuals will fill out a questionnaire with individual information, lifestyle habits, and medical history.
2. Biometric Data - Height and Weight, Blood Pressure, Waist Measurement, and Hip Measurement.
3. Blood Analysis - Fasting Lipid Panel and Fasting Glucose Serum.*
*LivingWell Health Solutions can provide additional tests for additional fees.
Data collected from the LivingWell Health Solutions HealthSCREENING will be used to create two individual, personal reports and one company report.
1. Health Report Card - A health risk profile that describes the level of risks for an individual.
2. Healthy Life Plan - A report that includes recommendations for health management activities the individual should undertake in order to properly manage his or her health risks and reduce his or her risk of illness over the next 12 month period.
3. The employer will receive a Personal Health Overview describing the overall health risks of the employee population. No individually identifiable health information is included in this report.
The LivingWell Health Solutions HealthSCREENING is the first step to engaging your employees in health improvement. The individual reports will make employees aware of their personal health risks and the Population Health Overview will make the employer aware of the overall health risks of the employee population. From these results, the employer will be able to determine the next steps that need to be taken to address their population's specific risks.
