LivingWell Health Solutions’ primary service, HealthONSITE, is proven to engage individuals in meaningful health improvement through an onsite clinic. Through the care of an onsite health care provider, employees are able to receive convenient, high quality care at their workplace. This service is free of charge to participants.
The service begins with a comprehensive overview of the program for both the employer and employees. LivingWell Health Solutions will establish and operate an onsite clinic that will allow employees the opportunity to receive the necessary services they need. This service is best suited for organizations that employ more than 300 employees.
HealthONSITE is an ongoing service that includes continuous program support. This support includes account management, guided program planning, employee orientations, annual Personal Health Evaluations, annual personal Health Report Cards, annual personal Healthy Life Plans, clinic set-up, clinician recruitment, assistance with implementation of a Financial Incentive System, on-going clinic management, continuous program reporting, compliance monitoring, and an onsite clinic.
Onsite health care providers are trained to work with individuals to help manage their emerging chronic conditions and and assist with lifestyle management including:
Diabetes
Hyperlipidemia
Hypertension
Triglycerides
Chronic Obstructive Pulmonary Disease
Tobacco Cessation
Weight Management
Clinicians provide chronic disease management, which is the area of greatest cost savings, as well as episodic care, as needed in the onsite clinic. The LivingWell Health Solutions program is not meant to replace a primary care physician, but to work in collaboration with the primary care physician.
