All personal medical information collected by LivingWell Health Solutions is private, secure, and confidential. The requirements for collecting, storing, and sharing medical information are given to each participant and available in the on-site clinics.

AT NO TIME WILL AN INDIVIDUAL'S PERSONAL HEALTH INFORMATION BE SHARED WITH THE EMPLOYER OR ANYONE ELSE UNLESS THE PARTICIPANT PRESENTS WRITTEN AUTHORIZATION TO LIVINGWELL HEALTH SOLUTIONS REQUESTING THE TRANSFER OF DATA.
