Health Risk Assessment Form

Living Well Health Solutions offers a flexible array of services to accommodate the uniqueness of potential client organizations to account for their different “stages of readiness” to manage health risk. Below is a description of the three levels of service offered:

• HealthAWARE: A health assessment service performed on-site at Employer’s office location(s). The specific services included are:
  

  >> Medical History and Lifestyle Survey

  >> Biometric Measurement

  >> Blood Analysis

  >> Health Report Card

  >> Population Health Report Card

• HealthDIRECT: All HealthAWARE service components are included in this level of service. In addition the following services are provided:

>> Healthy Living Plan (“H.E.L.P.”)
>> Written Reminders at Follow-up.
>> Intervention compliance with H.E.L.P Plan recommendations.

• HealthONSITE: All service components included in HealthAWARE and HealthDIRECT are provided at this level. In addition, on-site health professional services are provided in a convenient personal setting at Employer facilities. The provider option includes a Nurse Practitioner, Physicians Assistant, and/or a Health Education Specialist.
